If you choose to register with our service, IMCO collects certain minimal

number, social security number, e-mail address, and birth date, for identification
and billing purposes. It is optional for
you to decide whether to provide additional information beyond these
items. Please note that we collect
social security numbers as an option because healthcare facilities often use
patients’ social security numbers for identification purposes. IMCO also stores your Living Will and Health
Care Surrogate documents, along with related personal information.
We will not sell (except in a transaction involving the sale of all or
substantially all of the assets of IMCO), rent, lease, share or exchange the
information we store for our subscribers.
Your Living Will and Health Care Surrogate documents and personally
identifiable information are released only to those healthcare providers who
request such information and who are
authorized by you to do so.
When you enroll with IMCO, you will authorize us to release your advance
directive and related information to any authorized health care provider. IMCO makes these items available for
retrieval via email and fax. To receive
such documents or items, the authorized health care provider must provide your
identification number.
IMCO will use your e-mail address to contact you annually to update any changes to your Living Will or
Designation of a Health Care Surrogate.
Please note that when you register for our service or renew your subscription
online, your credit card information is processed via a certified secure
authority and is not stored by IMCO.
IMCO may choose to contract with third party service providers who assist in
the processing of subscriptions and/or maintenance of IMCO information. Such
service providers may be permitted restricted access to the information we
collect solely in order to provide their services. In such event, these service
providers are required to agree, in writing, that they will abide by the
privacy statement and privacy standards implemented by IMCO.
IMCO does not target children under the age of 13 and does
not offer or knowingly sell any of its products or services to children.
IMCO may disclose your personal information if we are required to do so by
law or in the good faith belief that such action is necessary to: (a) conform
to the law or comply with legal process served on IMCO; (b) protect and defend
the rights or property of IMCO; or (c) act in urgent circumstances to protect
the safety of the public or users of IMCO’s service.
Also, IMCO collects a minimal amount of contact information from those users
who complete the “Contact Us” form. This
information is strictly used to respond to requests for information or
questions by the users of this form. None of the required information and/or
optional information collected on this form is used for other purposes than
those stated in this policy; however, IMCO may use third parties to assist in
providing certain services to users. In the event that IMCO utilizes a
third party to provide such services, our contract with the third party will
include obligations upon the third party to keep all information obtained
during such contract confidential and secure.
Except as otherwise stated, we do not store any personally identifiable
information about visitors to our site. However, we may use this information in
aggregate form to track the number of visitors to our site. When IMCO subscribers
visit our web site to update subscription information, our website uses
"non-persistent cookies," which is a form of technology that a server
gives to your browser when you access our website or a section of the website. The use of non-persistent cookies allows you
to navigate our web site without being required to enter your password more
than once during your visit to our site. Also, this cookie is deleted once you leave
our website.
