- Complete the It's My Choice Online consent agreement that gives your express permission for health care providers to have access to your living will and health care surrogate documents. You may also register your existing Advance Directive Documents. Please click on the link below to obtain registration information.
- Once you have processed your one time payment of $30.00 by credit or debit card, you will be prompted to answer a series of questions that are designed to have you think about the kinds of medical treatment you may or may not choose to receive in a particular situation. These choices will instruct your health care designee to make these decisions in accordance to your wishes should you be incapacitated or otherwise unable to verbalize your wishes.
- It's My Choice Online, Inc. will produce your Living Will and Health Care Surrogate. You may print as many copies as you need for your records. You will be instructed to mail a signed copy of your new Living Will and Health Care Surrogate documents to It's My Choice Online, Inc. Your signed copy on file ensures the authenticity of your choices. Documents are marked as "pending authorization" until a signed copy is received by It's My Choice Online, Inc. Your Living Will and Health Care Surrogate documents will be stored in the national directory and be made available only to health care providers through our online provider access.
- You will receive a Identification Card stating that you have registered your Living Will and Designation of a Health Care Surrogate with It's My Choice Online, Inc. to carry in your wallet.
- You will be contacted annually to update any changes to your Living Will or Designation of a Health Care Surrogate. Keep in mind that it is not necessary to update a Living Will, however, It's My Choice Online, Inc. will give you the opportunity to do so if any changes occur.
